APPLICATION CHECKLIST
Completed applications should include:

  • Completed application form
  • Current CV (Please make sure your CV includes the following information:
    • Education
    • Dissertation or thesis
    • Awards, honors, fellowships, scholarships, grants
    • Professional experience
    • Publications, invited papers, exhibits, conference presentations, etc.
    • Teaching, research interests
    • Academic Service
  • Four letters of recommendation (completed and submitted on official letterhead) with recommendation forms from leaders in the field of public health who can attest to the applicant’s presence and capacity in the field of public health including: (a) dissertation chair (b) faculty advisor (c) community health professional and (d) applicant’s choice
  • Official, sealed transcripts from all schools attended (including the most recently completed quarter/semester of your current school

If selected for the award:

  • Verification of U.S. citizenship or permanent residency
